Job Description:
Care Coordinator RN - Buffalo Grove ICC (Hourly,
Days/Evenings)Shift: Hourly, Days and Evenings, Weekend as
needed
Reporting to the Director or assigned Supervisor, facilitates a
positive, collaborative work environment with an interdisciplinary
team approach. The Care Coordinator utilizes the nursing process in
the delivery of patient care including assessment, planning,
implementation and evaluation of patient outcomes. Provides
department supervision and oversees the staff scheduling functions
to effectively utilize resources, and modifies a patient's plan of
care as indicated. Resource to the healthcare team collaborating to
achieve optimal clinical, financial and satisfaction outcomes in
primary and specialty care. Assesses patients, facilitates plan of
care, implements and coordinates care progression, monitors and
evaluates options and services to meet an individual's health
needs. This job description was developed in accordance with the
Illinois Nurse Practice Act and the ANA Scope and Standards of
Practice.
1. Achieves quality patient outcomes by managing day-to-day care
coordination and clinical management activities. Models and
facilitates collaboration among the interdisciplinary team, i.e.,
physician, therapies, clinical nurse specialists, utilization
coordinators, and other departments. Collaborates with
interdisciplinary team to identify and resolve issues that impact
team performance and the delivery of patient care services.
Communicates and collaborates with others to provide continuous,
smooth transition of care and day-to-day operations. Coordinates
patient plan of care, based on area of service and role
expectations, to assure interventions are consistent, safe, timely
and appropriate. Assesses continuity of care and evaluates
coordination of interventions by the interdisciplinary team to
ensure consistency and compliance within the plan of care
recommendations.
2. Makes assignments and manages staff throughout shift promoting
continuity of caregivers. Monitors daily service activities,
patient flow, and facilitates completion of assignments and
deployment of staff. Provides oversight to ensure adequate unit
supplies, equipment, and stock. Utilizes supplies and equipment in
the provision of patient care in a manner, which assures quality
and cost effective patient care. Informs assigned Supervisor of any
service disruption involving staff, customer service, and/or
process improvement opportunities to better meet the needs of the
patient experience. Maintains a safe work environment in compliance
with all regulatory agencies.
3. Provides patient care as indicated and appropriate to the scope
of service area. Responsibilities may include, but not limited to
the following:), IM antibiotics, immunizations, administration of
nebulizer treatments, etc. Provide Nursing Visits and POC testing
and oversees clinical staff performing these functions. (i.e. Pro
times). Oversees site-specific CLIA and OSHA standards and has
oversight for daily QI processes to ensure customer service and
quality goals.
4. May also perform various OHS tests according to client
specifications. Reviews OHS client database to ensure that the
client company receives correct services for its employees.
Performs substance abuse specimen collection, i.e., Breathalyzer
testing (EBT), and Urine Drug Screens (UDS). Assists with other
ancillary testing or mandated exams according to identified
services and related guidelines. Processes Chain of Custody urine
drug screen specimens for shipment to certified labs for testing,
according to the client company's instruction.
5. Participates in evaluating quality and effectiveness of nursing
practice by participating in quality improvement. Incorporates
evidence based standards into nursing practice and modifies nursing
practice as indicated. Maintains professional knowledge in nursing
practice by participating in both mandatory and professional
educational initiatives. Maintains clinical competencies as defined
by areas of practice.
6. Interviews, hires, disciplines and discharges (when necessary)
subordinate team members. Regularly evaluates the performance of
team members in accordance with the hospitals performance
management system. Supports development of team member's knowledge
and skills through regular feedback, recognition of positive
accomplishments and coaching opportunities.
7. Interacts with and contributes to professional development of
peers and other health care providers as colleagues. Shares
knowledge and provides feedback to contribute to an environment
supportive of clinical education. Assigns or delegates tasks as
defined by State Practice Act according to knowledge and skills of
designated caregiver. Collaborates with physicians and
interdisciplinary team using open dialogue for the provision of
patient care.
8. Communicates and coordinates plan of care to patient and family
regularly respecting the patient's wishes to share. Communicates
patient data to physician and interdisciplinary team as indicated
to change/initiate interventions as needed. Documents collection of
patient health data, interventions and communications and is
responsible for Case Management.
9. Initiates patient and family teaching as soon as patient
demonstrates readiness to learn. Assesses patient's learning style
and learning barriers and modifies teaching plan accordingly.
Provides teaching on disease, management strategies, emergency care
and drug information when appropriate. Identifies community
resources to support patient self-management of disease.
10. Facilitates decisions and actions on behalf of patients in an
ethical manner by maintaining patient confidentiality, acting as a
patient advocate and assisting patients so they can advocate for
themselves. Seeks available resources to support ethical practice,
assists patient and family in becoming informed consumers about
cost, risk and benefits of treatment and care.
11. Responsible for oversight of patient electronic medical record
(EMR), Epic and Systoc documentation. Reviews clinical
documentation for completeness and accuracy.
12. Demonstrates the knowledge and skills necessary to assess,
treat, and provide care appropriate to the age(s) of the patients
served. Maintains clinical competencies as established by areas of
practice.
13. Adheres to all Northwest Community Healthcare standards,
policies, and procedures.
1. Successful completion of a BSN from an accredited school of
nursing.
2. Knowledge of managed care models financial reimbursement
systems, clinical case management processes and utilization
management issues.
3. Current licensure to practice nursing in the State of
Illinois.
4. Current CPR certification from the American Heart
Association.
5. Excellent verbal communication and interpersonal skills needed
to work in a collaborative practice setting with an
interdisciplinary team and to interact effectively with patients,
payers, reviewers and other agencies. The ability to adapt
communication approach with different members among the
interdisciplinary team.
6. The analytical skills necessary to understand the relevance and
impact of data and clinical information to support the appropriate
utilization of resources associated with the cost and quality of
patient care while keeping the focus on positive patient care
outcomes. Excellent skills in the following areas: Critical
thinking, problem identification and resolution, adoption to change
and negotiating
7. The leadership skills necessary to delegate appropriately and
direct the activity in provision of patient care. Ability to
project a professional image that is consistent with NCH and NCH
Medical Group value of customer focus.
8. The organization skills necessary to work independently and to
prioritize and effectively manage time and resources manage
multiple priorities simultaneously.
9. The ability to perform effectively and efficiently under the
pressure of stressful events or emergent situations.
10. Ability to perform computer skills necessary for EMR
documentation and orders, data inquiry, and data entry and
demonstrates working knowledge of all computer applications
necessary to complete documentation of patient services.
PHYSICAL REQUIREMENTS:
1. Requires light physical effort. May require frequent periods of
standing or walking. May require frequent lifting or moving of
lightweight material, occasional lifting of average-weight
material, rarely lifting or moving of heavy weight material.
2. The flow of work and character of duties requires normal mental,
visual and auditory attention all of the time.
3. Requires visual acuity adequate to assess patients and
read/record patient charts. Passing visual color discrimination
testing required.
WORKING CONDITIONS:
1. Conditions are acceptable. There is a possibility of exposure to
infectious diseases and biophysical hazards.
