Division Overview: The Adult Criminal Justice and Treatment Services (ACJS) supports clients who may have struggles related to a substance use disorder. We provide opportunities for clients to rebuild their lives so they can become healthy and self-sufficient. ACJS’ work includes screening and assessments for substance use disorders, placing clients into treatment programs, monitoring and reporting on progress, and providing support including, but not limited to, insurance enrollment, obtaining a state ID, and finding employment resources. We are currently looking for Full-Time – Peer Support Advocate Starting at $39,000 - $40,000; contingent upon experience, education, etc. Position summary: The Collaborative Bridges team is an innovative care coordination pilot program to connect individuals and families with substance abuse and mental health needs upon their discharge from hospitals and provide support care coordination services until linked to long term community providers or consumer needs are resolved. This position supports others in their recovery journey and serves as a role model, mentor, advocate and motivate individuals in order to help prevent relapse and promote long-term recovery. It is important that the Recovery Support Advocate has the ability to share personal recovery experiences and to develop authentic peer-to-peer relationships. Essential Duties and Responsibilities: Assists recovering persons to identify personal interests, goals, strengths, and weaknesses regarding recovery; educates and coordinates program services and resources Evaluates, facilitates, and monitors individual’s progress in relationship to established goals and objectives Locates and re-engages clients into recovery services including attending linkage appointments with clients. Ensures documentation and service hour goals are met on a regular basis, adhering to regulatory and compliance standards Supports the individual in transitioning from a professional directed service plan to a self-directed wellness plan Provides referrals for identified needs of the client and crisis intervention services Researches and connects with supportive services organizations and maintains a community supportive services database Collaborates with internal and external partners to coordinate and monitor client services Assists support staff with specimen collection as needed and maintains compliance with chain of custody protocols Our Ideal candidate: High school diploma or equivalent Experience participating in their own personal recovery process in order to care for self and/or family member Must understand and respect an individual’s unique path to recovery Must possess a commitment to the recovery community Demonstrates proficiency in Microsoft Office Suite Ability to work with a diverse population Within one year of hire, will be required to obtain Certified Recovery Support Specialist certification (CRSS) through the Illinois Alcohol and Other Drug Abuse Professional Certification Association Knowledge of community support resources, organizations, governmental agencies, and nonprofits Our benefits package includes: Medical/Dental/Vision/Life Insurance and Flexible Spending Paid Leave - STD/LTD Paid Time Off/Sick Time/ Floating Holiday Tuition Reimbursement 403 B (retirement plan) If you are interested in this position, please visit the TASC website at www.tasc.org and apply online.
