Job Description:
POSITION SUMMARY L’Arche Chicago is seeking an organized,
compassionate, and proactive Manager of Care to join our team and
community. This person will be responsible for ensuring that
appropriate and effective programs and services are developed and
implemented for the fifteen core members (adults with developmental
disabilities) living in L’Arche Chicago’s four residential group
homes (CILAs). The Manager of Care ensures that the core members
are affirmed, supported, and encouraged to grow with dignity and
respect in all areas of their lives by effectively coordinating
supports and resources for each person. The ideal candidate will
have a passion for supporting people with developmental
disabilities to achieve their full potential, as well as a
collaborative and community-oriented spirit. KEY RESPONSIBILITIES:
Oversee health care of core members, including coordinating with
community nurse for medication and health needs. Collaborates with
the House Managers to ensure core member medical appointments are
scheduled, completed, and documentation is completed. Maintain
health records and all State required documentation in each core
member’s electronic file (annual physicals, dentist visits,
up-to-date prescriptions, etc.). Complete annual ICAP and HRST
assessments for each core member. Submit any relevant state funding
requests such as 53R, 37U, or 36U or nursing packet updates. Lead
the process of writing annual plans for the core members and
oversee L’Arche Chicago’s role in implementing the plans, including
serving as the point of contact for the ISC. Support House Managers
in scheduling Assistants (DSPs), monitoring and approving PTO
requests, responding to support needs, and staying within budgeted
hours. Lead the Support Crew program, including a weekly meeting.
Support the scheduling of on-calls and serve as a weekly person in
the rotation. Lead a health orientation on core member health needs
for new Assistants. Coordinate behavioral support services, serving
as a liaison for our BCBAs and attend weekly support services
meetings with the BCBA. Be aware of and oversee follow-up of any
critical incidents and illnesses. Support each home and assist with
finding appropriate resources when any core member is in crisis.
Oversee the core members’ participation in job coaching, day
programs, and competitive employment, ensuring each core member is
adequately supported and has meaningful day programming. Serve as
the point of contact for day programs and ensure transportation
options are available (including UZURV, PACE, and Uber). Provide
direct service coverage and/or transportation in emergency
situations, where core members’ needs warrant, when staff shortages
occur, or when budget constraints necessitate. Participate in the
weekly assistants meeting and weekly Leadership Team meeting.
REQUIREMENTS: Embraces the mission and values of L’Arche; previous
L’Arche or other community living experience strongly preferred.
